If you would like your dental insurance to cover your new patient appointment, please provide insurance information 48 hours prior to the appointment. If you have booked your appointment within less than 24 hours of the appointment, please provide the information at the time of booking so that we can do our best to get your insurance verified (and have your insurance company directly cover your plan benefits).
If your insurance is not verified by appointment time, it’s no problem, but you will need to pay the appointment fee. If you’ve booked an appointment and need to submit your insurance information, please email your name, date of birth, subscriber’s name and date of birth (if you are not the primary on the plan), insurance company’s name, insurance company’s phone number, insurance ID number (or your social security number), and insurance group number to email@example.com.
Again, upon booking your appointment we offer a complimentary benefits check, estimate your out-of-pocket cost, and submit the insurance claims on your behalf.
